I think with fantasy, it would be something that is like a treat or addition to the usual stuff.
Like a married couple might have a 3some fantasy,


I think it is not a fetish until it reaches the point where you think about it every single time you get off. Of course, if you absolutely NEED it to get off...that would be a fetish, I think.

Also why fetishes often 'develop' over time...

I think it has more to do with specific unrealistic situations. Because you could mention the fantasy of doctor/teacher/nurse, however -- one could argue that would be a fetish also... if someone NEEDS that to get off, right?
